  • Depaul USA Respite Center of
    of its kind in Philadelphia Respite clients work in collaboration with staff to develop medical and housing plans To support disease self management and recovery the respite provides rest recuperation medication management and case management Depaul USA also provides comprehensive residential services including 24 hour supervision meals and basic shelter services Our partner PHMC will provide a Registered Nurse and Licensed Practical Nurse at non traditional hours to manage health services and provide case management to help respite clients establish medical homes and follow treatment regimens What does success look like Starting with the center s opening in 2014 the respite house will track short term outcomes for clients One goal of the respite is to demonstrate cost savings in addition to improved health outcomes Other areas for tracking include discharge to stable housing improved access to a medical home and improved experience of care Initially data will be tracked and reported monthly to respite partners in order to close the feedback loop Why is this a critical part of the community Homeless adults are hospitalized more frequently than those in the general population and often require longer in patient stays however their lack of a stable home environment diminishes the long term effectiveness of their hospital care Philadelphia s most vulnerable homeless have an increased risk of death despite the availability of high cost health services in the region Currently homeless individuals are discharged from the hospital to a shelter or the street frequently without medications treatment medical follow up or recovery support This often results in hospital readmission and the progression of chronic physical and behavioral health problems There is a growing consensus among stakeholders in the community there is a need to improve the transition of homeless people from hospitals back into their community Medical respite is

    Agencies Depaul USA partners with different agencies and community groups to find solutions to chronic homelessness We recognize the value of forming partnerships and appreciate the opportunity to work with other agencies to fulfill our mission Our partners include Meeting Alliance This full service meeting and event management company has contributed goods services and volunteer time to Depaul House Nutritional Development Services of the Archdiocese of Philadelphia NDS part of the Archdiocese of Philadelphia an agency that provides food to organizations serving the poor collects and donates food to Depaul House on a regular basis The Philadelphia Office of Supportive Housing Much of the food for residents at Depaul House comes from the Office of Supportive Housing OSH Emergency Food Distribution Program OSH assists individuals and families in moving towards self sufficiency and independent living through Philadelphia s homeless continuum of care SEI SEI a leading global provider of asset management investment processing and investment operations solutions has contributed goods services volunteer time to Depaul House SHARE Food Program Food is donated on a regular basis to Depaul House by SHARE the Self Help And Resource Exchange SHARE is a non profit organization that serves a regional network of community

    50 65 frequently fall between the cracks of governmental safety nets while not technically old enough to qualify for Medicare their physical health assaulted by poor nutrition and severe living conditions may resemble that of a 70 year old Connections to primary care and community services at the client s pace can help stabilize and improve lives while reducing the burden on the crisis care system Eligibility Criteria for St Raymond s House Must currently be homeless Single men and women with chronic health conditions including diabetes hypertension COPD Self referrals will not be accepted must be referred by an authorized referral agent case manager in a social service agency serving homeless clients Must have no sexual or violent criminal convictions If you are interested referring someone for services to St Raymond s House please contact us at 215 298 9219 Why this project The population experiencing homelessness is aging This impoverished population often has acute and chronic health needs and too often individuals depend upon the emergency room for health care This crisis care is not sufficient for resolving their health needs which continues to become a greater burden on the health care system and which prevents individuals from

    Agencies Partner Agencies Depaul USA s Daybreak day resource center is designed on a partnership model It is a place where different agencies and programs collaborate and share resources For example Goodwill Industries has an office at Daybreak to assist homeless individuals attain employment The Saint Vincent DePaul Society provides critical financial assistance to help restore utilities and defer housing costs As a strategic goal Georgia College offers primary health care with the goal of making the medical clinic inside the Daybreak program the medical home for homeless men and women Staff coordinate the case management for program participants and develop additional programming with the homeless men and women who come to the center This Depaul program is the result of a unique collaboration with representatives of Macon s faith communities who came together to address homelessness in Macon Representatives of the city s Baptist Catholic Episcopalian Presbyterian Jewish Methodist and Unitaria n Universalist congregations have all committed funding to support Daybreak s day to day operations Also numerous commercial collaborators are donating such as Armstrong World Industries and Cox Communications Additionally The Rotary Club of Macon chose Daybreak to be the recipient of their donations and GA College State

    sufficiency In New Orleans Depaul USA provides access to the St Vincent DePaul Free Pharmacy and the Vision Center Additionally our Support Specialist works with clients to assess refer and transport clients to necessary services available throughout the community Beginning in 2014 Depaul USA began the rapid re housing program moving individuals who have faced years of homelessness into apartments These 15 units scattered throughout the city give individuals the ability to settle into a stable lifestyle and rebuild lives of greater dignity How Our Van Makes a Difference in New Orleans During 2013 430 individuals were supported with a transportation and case management program The van made 152 trips travelling more than 2 400 miles Of these individuals 168 were transported to various agencies for services 235 received medications 56 had eye exams and or received glasses and 42 participated in housing appointments The History of the Project Hurricane Katrina destroyed many of the services that assisted the homeless while simultaneously increasing the homeless population After Katrina the Rebuild Center at St Joseph Parish brought together the collaborative efforts of the Harry Thomson Center operated by the Jesuits and the Presentation Sisters Lantern Light project At the Rebuild Center
