    50 65 frequently fall between the cracks of governmental safety nets while not technically old enough to qualify for Medicare their physical health assaulted by poor nutrition and severe living conditions may resemble that of a 70 year old Connections to primary care and community services at the client s pace can help stabilize and improve lives while reducing the burden on the crisis care system Eligibility Criteria for St Raymond s House Must currently be homeless Single men and women with chronic health conditions including diabetes hypertension COPD Self referrals will not be accepted must be referred by an authorized referral agent case manager in a social service agency serving homeless clients Must have no sexual or violent criminal convictions If you are interested referring someone for services to St Raymond s House please contact us at 215 298 9219 Why this project The population experiencing homelessness is aging This impoverished population often has acute and chronic health needs and too often individuals depend upon the emergency room for health care This crisis care is not sufficient for resolving their health needs which continues to become a greater burden on the health care system and which prevents individuals from

    Agencies Partner Agencies Depaul USA s Daybreak day resource center is designed on a partnership model It is a place where different agencies and programs collaborate and share resources For example Goodwill Industries has an office at Daybreak to assist homeless individuals attain employment The Saint Vincent DePaul Society provides critical financial assistance to help restore utilities and defer housing costs As a strategic goal Georgia College offers primary health care with the goal of making the medical clinic inside the Daybreak program the medical home for homeless men and women Staff coordinate the case management for program participants and develop additional programming with the homeless men and women who come to the center This Depaul program is the result of a unique collaboration with representatives of Macon s faith communities who came together to address homelessness in Macon Representatives of the city s Baptist Catholic Episcopalian Presbyterian Jewish Methodist and Unitaria n Universalist congregations have all committed funding to support Daybreak s day to day operations Also numerous commercial collaborators are donating such as Armstrong World Industries and Cox Communications Additionally The Rotary Club of Macon chose Daybreak to be the recipient of their donations and GA College State

    was a shot in the dark we re dealing with some of the toughest clients out there and the guys were getting bigger and bigger I asked my colleague are there any words or wisdom or choice phrases you have Three words I say to the guys dedication motivation and focus Be dedicated to the task motivated to do it and focused while you re doing it He laughs I write it down for them He says that heeding these three words provide for the consistency that leads to results While Joe specializes in simple practices related to physical health he sees greater impact Not only has their mental illness improved but a lot of have lowered blood pressure not taking insulin anymore There are several guys who are diabetic who just watch their diets now There are more than 10 off of the high blood pressure meds because of exercising and weight loss Exercise is therapeutic We all have stress What they ve learned from working out consistently they don t need that beer or hit on the street they ve leveled their stress out with their meds and exercise All of the guys have reduced use of meds for mental and physical conditions One guy who is HIV positive when he started walking to support his health he started going to church too Then he met another walker Different activities and hobbies attract different company Before they were hanging around with other guys who weren t a good influence on them Once they start feeling good then I never saw the thugs again because the residents were out and about This walker now volunteers at two of the places where he used to get handouts I think that is neat too that s pretty cool now he gives

    Daughters of Charity are part of the Vincentian Family and Depaul USA s connection to the Vincentians and its international work really appealed to Sr Mary Fran Like the Daughters of Charity Depaul USA is committed to working with the poor in a practical and non judgmental way and responding to need through action and innovation In early 2010 Sr Mary Fran was invited to do a workshop on the Vincentian mission vision and values for the staff at Depaul USA She remarked I really enjoyed the workshop I realized that the staff was already imbued with the mission vision and values and I was able to help them articulate their thoughts Sr Mary Fran began volunteering at Depaul House in November 2010 four days a week usually from 3 00 7 00 p m It was important for me to become a member of the community so I can do what I need to be doing I wanted to stay in touch with the population that Depaul House serves Depaul House is in many ways a residence a place where people have their own space And the staff make the men feel they are at home at Depaul House When asked how Sr Mary Fran spends her time at Depaul House she explained I do anything that is needed I greet the men I talk to them sometimes I make supper for them I am in a privileged position because I get to know a side of life I wouldn t know otherwise Part of the constitution of the Daughters of Charity is to have mutual sharing and learn from the people that we serve and have them learn from us The men are the most gratifying part of my time at Depaul I have learned from them what
