    to provide support After these parents met informally to discuss their mutual concerns it was very apparent to them that the present models of service delivery were inadequate to ensure a safe and secure future for their children It also became clear that the issue of safe and secure future affected all families with children with disabilities regardless of the age of the parent Younger parents too were interested in securing the future for their son or daughter Interest from families with relatives with a disability was strong and enthusiastic They were eager to develop models of service which guaranteed safety and security and was independent of government policy and funding After three years of research and after successfully securing grant monies PLAN was born in 1989 PLAN recognized the vital and indispensable role parents have played historically in creating and developing services for our relatives with a disability PLAN honors the work and vision of all families who contributed to the development of supports and services which exist for persons with disabilities today PLAN S CORE VALUES Family Leadership Safety and Security through Relationships Self Sufficiency Contribution equals citizenship PLAN believes in the resourcefulness of communities and focus on

    Family Link s core values is safety and security through relationships Family Link facilitates the formation of relationships in the lives of individuals with disabilities by developing Personal Networks for them The real strength of these caring relationships comes when people are connected to each other Imagine a spider s web The strands extend from the center of the web to the edge Imagine if there was nothing else holding them together They would flap in the wind Their functional value would be minimal They need to be linked with each other in order to form the web The web is strong when all strands are interconnected It s the same for our family members relationships When the people in a personal network are connected with each other the web of support begins to approximate the thoroughness with which we care for our loved ones The core of Family Link s work is nurturing and maintaining networks of caring relationships around our loved ones Networks of friendship and love aren t just something for down the road They are crucial right now They give lives meaning fun and fulfillment They create a place for sharing life passions as well as

    we count on confide in and people who care about us They are our family members friends and acquaintances Unfortunately many people with disabilities don t experience such support They need extra help to develop and maintain a network of friends and supporters A Personal Network is a group of caring dedicated people who are committed to maintaining a relationship with a person who is isolated and vulnerable by reason of disability The network becomes the eyes ears arms legs and heart of the person s life The network serves as informal monitors of the quality of life Personal Network members voluntarily form a relationship with the person with a disability and with every member of the Network Each member offers support advocacy monitoring and companionship Personal Networks can accomplish many things if the Networks are rich vital and continually evolving They pulse with their own life and have tremendous creative power Here are some of the many things a Personal Network can accomplish Prevent abuse and neglect Help the person find employment and arrange job supports Permit other family members to take vacations worry free Celebrate birthdays and holidays Share hobbies and other interests Engage the person in cultural

    wishes of the individual and their family to create a Personal Network that truly supports everyone involved The Facilitator must have special abilities and community awareness so that the people who participate in the Personal Networks are a good match with the person who is being focused on They help community members recognize the gifts of the individual and recognize that we all have abilities to contribute to society whether one has a disability or not It has been proven that without the presence of a skilled Facilitator it is typical that a Personal Network has very little chance of survival Family Link is committed to making sure that Personal Networks meet regularly and that they are large enough to provide continuous support Family Link charges an hourly fee for the services of the Facilitator On average 40 hours are required over eight months to develop a network Established networks require an average of 24 hours a year of a Facilitator s time to maintain the network If you are interested in our services or if you would like more information about becoming a Network Facilitator please contact Cassie Higgins at 720 266 2791 or 303 527 0627 X 791

    to provide the family with A set of goals A six month work plan A list of potential network members After this the family can decide whether they wish to continue with the formation of the network This Exploration phase take about 10 hours to complete Step 2 Formation This is a time of group development The focus of this phase is on the recruitment of network members and having network meetings The family can expect to have 2 4 network meetings over a 6 to 8 month period and regular contact with their facilitator The Facilitator extends invitations to future network members and at each meeting the goals of the individual are reviewed and discussed Strategies are then developed and commitments are obtained from the network members so that these goals can be achieved It is also during this phase the strength of the network will be determined and long term members will be identified Some networks are self sufficient at this point while others require more development and recruitment A second work plan is then developed for the family outlining the goals for the next 6 months The average number of hours in this phase is 30 hours
