    to engage in collaborative discussions planning and action with various entities vested in the health of a community Participants will also have an opportunity to learn about the Community Health Living Index CHLI tools and how the tools can be used collaboratively within a community to discover strengths identify needs plan for action and ultimately build healthier communities The presenters will share examples of how communities have successfully collaborated to influence policy and systems change Objectives As a result of participating in this Webinar participants will be able to 1 Identify meaningful ways for hospitals local health departments and communities to collaborate in community health planning and implementation 2 Understand the challenges and opportunities for col laboration presented by the Affordable Care Act s definition of Community Benefits and IRS requirements 3 Describe how the County Health Rankings can inform a community health assessment CHA or community health needs assessment CHNA 4 Define the Community Health Living Index CHLI 5 Describe how CHLI can be used to assess a community s support for healthy living and to develop an action plan 6 Share examples regarding how community health planning can be used to influence policies and systems Target Audience This webinar is intended for public health practitioners hospital staff involved in Community Benefits programs and members of local coalitions collaborating with partners to assess and improve their communities Presenters Vondie Woodbury Vondie Woodbury is Director of Community Benefit for Trinity Health System She is also Director of Community Benefit for Mercy Health Partners Muskegon MI and Executive Director of the Muskegon Community Health Project One of the oldest community health networks in the nation MCHP has been recognized for community innovation by the New York Times and Wall Street Journal Prior to MCHP Vondie served for 14 years on the staff of U S Senator Donald Riegle as Director of Michigan Operations She is a founding member and immediate past Board Chair of Communities Joined in Action She recently received CJA s Mary Lou Andersen Lifetime Achievement Award which recognized her work in local community health improvement A graduate of Grand Valley State University and Western Michigan University Vondie lives in North Muskegon Michigan Ann W Marchetti MS For the past eighteen years Ann Marchetti has held leadership roles in several successful nonprofit agencies towards a singular goal to improve the health and well being of children youth and families Drawing upon her unique combination of public policy business and creative skills Ann has assisted these nonprofits in optimizing their business models to build organizational capacity and develop sustainable programs Ann was awarded the 2008 Distinguished Service Award for Outstanding Service to Children and Youth by the National Child Labor Committee This body remarked Ann has been at the forefront of action and has been the voice to improve the health education well being and futures of millions of children Most recently Ann served as the Executive Vice President and Chief Operations Officer for the Cambridge MA based National Initiative

    Environmental Change Strategies Learning Collaboratives Obesity Action Roadmap Accreditation and Mock Review Collaborative QI Learning Collaboratives CCCD Archived Projects Health and Information Technology Data Across Sectors for Health DASH Illinois Framework Stakeholder Engagement Project IMPACT Provider Enrollment Communications BIP Stakeholder Engagement CHIT Archived Projects About IPHI Board of Directors Staff Directory Staff Representation Funders Positions available at IPHI Contact Us Public Health Links Privacy Policy Reports Publications You are here Home Center for Community Capacity Development CCCD Archived Projects Accreditation Accreditation The Illinois Accreditation Development Project IADP was funded from 2005 to 2011 by The Robert Wood Johnson Foundation RWJF IADP was part of the Multi State Learning Collaborative through which funded states shared their experiences in performance measurement capacity assessment and quality improvement in public health agencies In Phase One of the project the IADP designed a voluntary accreditation framework to enhance the Illinois local public health certification program and develop a more performance based accreditation program In Phase Two the IADP created and tested systems tools and protocols for a potential Illinois Accreditation Program to ensure the new program is focused on quality improvement Beginning in April 2008 Phase Three of the IADP focused on specific quality improvement targets the application of QI techniques and further refinements in the proposed voluntary accreditation process The Project was governed by the Illinois Accreditation Development Task Force that included representatives from the following organizations Illinois Department of Public Health Illinois Department of Human Services Illinois Association of Public Health Administrators Northern Illinois Public Health Consortium Southern Illinois Public Health Consortium Illinois Association of Boards of Health University of Illinois at Chicago School of Public Health Illinois Public Health Association The 2009 Illinois Accreditation Task Force was staffed by the Illinois Public Health Institute Click here for a complete list of members
