  • Health IT Investments Should Enable People to Download Their Own Information at the Click of a Blue Button | Markle | Advancing America's Future
    like summaries of doctor visits medications you are currently taking or test results Being able to have your own electronic copies and share them as you need to with your doctors is a first step in truly enabling people to engage in their health care said Carol Diamond MD MPH managing director at Markle This capability is not common today but we have the opportunity to make it a basic expectation especially now that billions of public dollars will be flowing to help subsidize health information technology Diamond said The paper is being released as Medicare and the U S Veterans Administration VA prepare to implement a blue button this fall that will for the first time allow beneficiaries to electronically download their claims or medical information in a common format from the My Medicare gov and My HealtheVet secure websites President Obama announced the blue button for veterans in an August 3 address For the first time ever veterans will be able to go to the VA website click a simple blue button and download or print your personal health records so you have them when you need them and can share them with your doctors outside of the VA the President said The Markle collaboration s recommendations are timely because the American Recovery and Reinvestment Act requires that individuals be able to receive electronic copies of their records from providers electronic health record systems In addition new federal economic stimulus rules require health care providers and hospitals to deliver electronic copies of things like lists of medications after visit summaries and lab results if they choose to participate in federal subsidies for using health information technology The Markle policy recommendations reflect consensus on one means by which this can be accomplished today securely and efficiently The group details privacy policies and practices for implementing the download capability with sound authentication and security safeguards and suggests practices to help individuals make informed choices about downloading their information For example it recommends specific language to remind individuals not to download or store their personal health information on shared computers Christine Bechtel vice president of the National Partnership for Women Families and a member of the federal Health IT Policy Committee supported the Markle policy paper People see a lot of different health care providers over time so giving them a convenient option to securely assemble their health information from multiple sources will help them better manage and coordinate their own care Bechtel said This capability is one of the simplest and most direct ways of helping patients and families see the benefits of the federal health IT investments that they as taxpayers have helped fund The proposed privacy policies build on the Markle Common Framework for Networked Personal Health Information a set of recommended practices for individual access to information and privacy The framework first released in 2006 is widely supported by a range of technology companies insurers provider groups and consumer and privacy advocates We recommend specific privacy policies

  • New Markle Survey Finds US Public and Doctors Alike Support 'Blue Button' for Downloading Health Information | Markle | Advancing America's Future
    Survey on Health in a Networked Life 2010 the first of its kind to compare public and doctor priorities for health information technology will be previewed at the Health 2 0 conference this week in San Francisco Seventy percent of the public and 65 percent of the doctors agreed with the concept of a blue button that you can click to download your own health information said Carol Diamond MD MPH Managing Director at the New York based non profit Markle Foundation Medicare and the U S Department of Veterans Affairs VA are preparing this fall for the first time ever for beneficiaries and veterans to have a blue button to download their claims or medical information from the MyMedicare gov and My HealtheVet secure Web sites With U S veterans now gaining access to a blue button and Medicare beneficiaries soon to follow these surveys indicate solid majorities of support among the public and doctors polled Diamond said This represents a remarkable agreement between the general population and doctors Diamond said In fact only 7 percent of the public and only 15 percent of the doctors disagreed with a statement that patients should be able to download copies of their pertinent health information The new Markle survey offers a first ever comparison of public and physician views on key issues surrounding health information technology including their views on the new federal incentives to stimulate use of health IT Different thematic elements of the findings will be released in coming months The new federal rules require health care providers and hospitals to give patients electronic copies of their lists of medications after visit summaries lab results and other pertinent information in order to qualify for an estimated 27 billion in federal subsidies for using health information technology The program now being rolled out as required by the Recovery Act is designed to make the sharing of patient information more efficient Sixty two percent of the public and 49 percent of the doctors surveyed said that for the billions in dollars of federal incentives to be well spent it s important for the program to set requirements that participating doctors and hospitals supply patients with electronic copies of their personal health information Only 8 percent of the public and 21 percent of the doctors felt that providing electronic copies to patients was not an important requirement for the program But overwhelming majorities of the public and of doctors surveyed also want to make sure the money comes with privacy safeguards As with our past surveys we found that privacy protections are a high priority Diamond said When asked about requirements necessary to make sure that federal incentive money for health IT would be well spent more than 80 percent of both the public and doctors said privacy safeguards were important Despite the strong support for the idea of downloading personal health information 83 percent of the public said they never ask for their information in electronic format The vast majority of

    Data Downloads into Helpful Tools Publication Date October 7 2010 SAN FRANCISCO The Markle Foundation and the Robert Wood Johnson Foundation s Pioneer Portfolio today recognized a set of innovations that convert simple health data downloads into useful tools to help U S veterans improve their health The foundations sponsored the Blue Button Developer Challenge to stimulate innovation and encourage doctors hospitals pharmacies insurers and personal health information services to let their customers securely download their medical information at the click of a blue button The US Department of Veterans Affairs and Medicare have led the way by preparing to offer a blue button on their password protected online portals to give veterans and beneficiaries the option of downloading copies of their medical or claims information Downloading your information is only the first step to making use of it said Carol Diamond MD MPH managing director at Markle The real winner of this challenge is the consumer because so many different approaches and ideas emerged to help patients put their information to good use Adobe s Blue Button Health Assistant received the top prize from a distinguished panel of judges Craig Newmark founder of craigslist org Steve Findlay MPH Senior Health Policy Analyst Consumers Union James Ralston MD MPH Associate Investigator Group Health Research Institute and Steve Downs of RWJF The foundations and the judges recognized the other finalists Microsoft MedCommons and RememberItNow I was impressed that the challenge finalists took fairly unfriendly raw data and made it readable usable and easy to share in different ways said Steve Downs assistant vice president Health Group Robert Wood Johnson Foundation Within the very tight time frame of this contest we are just scratching the surface of what is possible I m looking forward to tools that we haven t even imagined For the challenge Markle and Robert Wood Johnson Foundation asked participants to develop a web based tool that uses test blue button downloads from the VA or Medicare to help patients stay healthy and manage their care This competition was a great reminder that we each should be actively engaged in our own health and that there s great potential in online tools to help us make sense of our information Newmark said As we have seen with other technology a relatively simple step like making your health information downloadable as the VA and Medicare are doing can be revolutionary Ralston an internist and pioneer in using electronic personal health records PHRs to engage patients said The contents of the blue button download provide a good foundation for tools that help patients share information with various providers I was pleased to see many of the entries focus on communication of critical information between doctors and patients Findlay of Consumers Union emphasized not only the criteria for usefulness and usability but also the privacy requirements of the challenge Many consumers want convenient secure access to their personal health information and connections to their providers but only when their privacy and security

    on the Internet should be controlled According to the survey few Chinese Internet users are aware of government web sites despite the government s increasing investment in such projects Many Chinese believe that the Internet will increase political transparency 48 percent of Internet users believe that by going on line the Chinese will learn more about politics and 60 of users believe the Internet will provide more opportunities for criticizing the government The survey found that early adopters of the Internet in China are younger than 30 employed single live in urban areas and have higher than average levels of income and education University professors other educators and college graduates are using the Internet heavily Eighty five percent of the content which Chinese Internet users explore originates in China At present Internet users make up only eight percent of the population of mainland China indicating there is still much room for growth in the use of the Internet in China according to the survey Chinese use the Internet to seek either entertainment or information about entertainment as well as to communicate with like minded people on line Chinese Internet users indicate that they go on line for news more than for anything else but that much of the news they seek is related to entertainment In China to date the Internet has become an entertainment and communication highway but not an information highway said Professor Guo Liang of the Research Center for Social Development of the Chinese Academy of Social Sciences of Beijing who conducts the survey with the support of the Markle Foundation a New York based philanthropy which focuses on information technology Mainland Chinese use the Internet more for entertainment and chatting than for seeking information or news or for working or studying added Professor Guo at the Brookings Institution in Washington DC where he released the English language version Surveying Internet Usage and Impact in Five Chinese Cities this morning The Internet is profoundly transforming China s economy and society said Jeffrey Bader director of The Brookings Institution China Initiative Guo Liang is one of the foremost experts in China both on the technical aspects of the Internet s operation and on its impact on Chinese life We are delighted to have him come to Brookings to present the findings of his study The survey also tracked the kinds of information Chinese seek on the Internet and how much they trust such information While 83 5 of Internet users seek information on the web the majority of that information pertains to entertainment not traditional news Only 48 of users believe that Internet content is reliable and majorities of Chinese believe that certain kinds of content on the web should be controlled including the following categories of information pornography 84 7 violence 72 6 and junk messages or spam 51 9 Among other findings Professor Guo and his colleagues identified the following Eighty percent of the surveyed individuals younger than 24 years of age use the Internet and

    Media Releases Member Commentary President s Letters Videos Joint Statement on Meaningful Use Rule Publication Date July 13 2010 WASHINGTON DC The Markle Foundation the Center for American Progress and the Engelberg Center for Health Care Reform at Brookings today emphasized the importance of new federal rules on health information technology The U S Department of Health and Human Services today released regulations on implementing an incentive program for Meaningful Use of electronic health records EHRs The program authorized under the 2009 economic stimulus Recovery Act represents the largest investment in modernizing health information technology in U S history These regulations provide a promising foundation for encouraging the effective use of health information to improve patient care With coming payment reforms these incentives will give doctors and hospitals needed support for improving health care quality and efficiency said Mark B McClellan MD PhD Director Engelberg Center for Health Care Reform and Leonard D Schaeffer Chair in Health Policy Studies at the Brookings Institution The two words are critical together meaningful and use said Carol Diamond MD MPH managing director of the Markle Foundation The final rule has added flexibility to encourage providers to participate in the first phase of this critical effort to improve health promote efficiency drive innovation and protect privacy Now providers and patients can strive for meaningful use of modern information tools This marks a new beginning in the effort to infuse the health sector with the power of information said Peter Basch MD senior fellow at the Center for American Progress In the long run there is much more to be done to align quality and efficiency in care through health care and payment reform Basch said But today this incentive program provides a vital boost for doctors and hospitals to invest now in health IT and its optimal use for improving outcomes coordinating care and engaging their patients in health improvement For the past 18 months the three organizations convened a broad collaboration of experts and organizations to advise HHS on the new rules focusing on setting ambitious but achievable targets for the following priorities Establishing goals and metrics in terms of improving health and efficiency not mere purchase of technology Embracing patient engagement as a key aspect of Meaningful Use Adopting a phased approach to allow for technology development and testing at initial stages Prioritizing requirements for reporting quality results to avoid unnecessary burdens on physicians and hospitals Shaping technology requirements in a way that encourages innovation We congratulate the many people who worked on the essential details to govern this program Markle the Center for American Progress and the Engelberg Center at Brookings said in a joint statement The requirements must be ambitious enough to make the investments worthwhile but not so onerous that they discourage large numbers of doctors and hospitals from participating We look forward to reviewing the final rule in detail The Markle Foundation works to realize the full potential of information and information technology to address critical public needs

    Collaboration The Lawfare Blog Library Quick Links Our Book America s Moment Archive Media Releases Member Commentary President s Letters Videos AMA Markle Foundation Present PHR Survey Research at HIMSS Publication Date March 3 2010 ATLANTA GA The American Medical Association AMA and the Markle Foundation joined together to conduct coordinated surveys that examine physician and patient views of electronic personal health records PHRs Selected results of these surveys are being presented today at the 2010 HIMSS Conference in Atlanta Our surveys over the years have found that large numbers of consumers see the benefits of accessing their health information and connecting to health services online Our work with the AMA validates that many doctors also see the potential of this rapidly evolving technology said Carol Diamond MD MPH managing director at the Markle Foundation By asking physicians and patients comparable questions about PHRs we see that patients and physicians have some similar and some contrasting views Members of both groups often agree on the potential of PHRs to help individual patients and their families better manage their care but many doctors also express important concerns about PHR use like privacy and accuracy of data said Matthew Wynia MD MPH director of the Institute for Ethics at the AMA Four out of five Americans believe using an online PHR would provide major benefits to individuals in managing their health and health care services with 86 percent or higher saying that PHRs could help them avoid duplicated tests keep doctors informed move more easily from doctor to doctor check the accuracy of their medical records and track personal health expenses While less than 20 percent of physicians have significant experience using PHRs half agree that PHRs can help empower patients to participate in their care and 44 percent are willing to make use of PHRs as part of their clinical work Both patients and physicians cited privacy as a chief concern with PHRs Of those surveyed 87 to 92 percent of consumers said that privacy protection would factor into their decision to use an online PHR Similarly 70 percent of physicians agreed that PHRs may not have adequate privacy protections Other concerns cited among physicians include the possibility that PHRs could contain inaccurate information 79 to 85 percent were concerned about this create liability risks 76 percent and that physicians would not be reimbursed for time spent reviewing PHRs 63 percent The great majority of physicians haven t used PHRs yet so these concerns reflect expectations more than current practice Dr Wynia said These surveys can help us understand and proactively address physicians and patients hopes and concerns about PHRs which is critical to moving forward and using these new tools effectively said Dr Wynia Today s presentation provides some interesting glimpses into attitudes among doctors and the public as health IT tools become more widespread and much more study is needed said Josh Lemieux director of personal health technology at Markle It s increasingly important to understand how individuals use

Information Age Senator Gorton also served on the 9 11 Commission In the wake of the attempted Christmas Day attack on Flight 253 it is essential to distinguish between amassing dots and connecting them they told the Judiciary Committee in written testimony in connection with today s hearing entitled Sharing and Analyzing Information to Prevent Terrorism Information sharing is a means not an end The end goal is production of actionable intelligence derived from a form of collaboration that leads to insight and action The Markle Task Force is composed of national security policymakers from President and Congress to take swift action to ensure that policy makers have the best information available to confront a stark set of 21st century national security challenges The Markle Task Force is calling for immediate action to prepare America for the future by addressing the cultural institutional and technological obstacles that prevented the government from taking full advantage of information about the attempted Christmas Day attack Specifically the Markle Task Force offered five proposals to the Committee strong sustained leadership from Congress and the President transformation of how government does business to create a decentralized information sharing framework that enables collaboration not only between computers but among teams of people development of discoverability so that relevant information can be discovered and located quickly and efficiently to facilitate quickly piecing information together discoverability should be combined with a standard of authorized use to determine whether a user is authorized to see information that has been discovered development of government wide privacy and civil liberties policies for information sharing to Directors NEW YORK NY Following its Board of Directors meeting on Tuesday September 29 2009 the Markle Foundation announced that Slade Gorton former United States Senator from the state of Washington broader health care reform a group of three influential non profit organizations said today Committing to specific ambitious targets like preventing 1 million heart attacks and strokes and cutting Brookings Institution By tying support for health IT directly to achieving these quality and cost improvements the new health IT payments can be a critical step toward achieving this vision The group urged public and private sector leaders to embrace the 2015 health care improvement goals presented last month by an advisory committee of the U S Department of Health and Human Services HHS Office of the National Coordinator for Health IT ONC The goals include preventing 1 million heart attacks and strokes cutting rates of medication errors preventable hospitalizations and racial ethnic disparities in diabetes control in half and providing patients with easy access to their own health information The group proposes adding two 2015 goals to specifically address growth in health care costs identifying duplicative health care services and reducing them by 50 percent and halving the time spent by physicians on administrative tasks These goals are ambitious but achievable if we implement health IT and health reform effectively said Todd Park Senior Fellow at the Center for American Progress Top public and private sector leadership should firmly embrace these goals and facilitate their realization through communications to their own organizations and the American public We should commit to the achievement of these goals as we dive into the details of policy formulation and execution The three organizations have submitted a collaborative comment on behalf of a broad range of groups urging HHS to structure the criteria for ARRA s health IT subsidies to doctors and hospitals in a way that is most likely to achieve these key 2015 goals Success will be measured in the number of lives saved the quality of health improved and unnecessary costs controlled help consumers better manage their health 56 diverse organizations said today as they embraced a framework for personal health information access and privacy Consumers need to be full participants in modern health information tools and services to help them prevent illness communicate better with clinicians understand costs and treatment options make better health decisions and take better care of loved ones the group said in a joint statement Health reform requires putting the power of information at the fingertips of 300 million consumers said Zoë Baird president of the Markle Foundation which convened the group If we do this right consumers will contribute to a health care system that rewards quality slows an unsustainable spiral of costs and protects the privacy of sensitive information The recently enacted American Recovery and Reinvestment Act establishes billions of dollars in incentives for clinicians and hospitals to use health information technology including electronic health records EHRs The law also clarifies that individuals have the right to receive copies of personal health information from EHRs in electronic formats and authorize their information to be stored in a service of the individual s choosing In the age of the Internet there is vast potential for consumers to connect online to new services to make their lives easier and healthier said Democracy and Technology said Sound policies are key to public trust without which we will not see the benefits of health IT investments Consumers and clinicians will not participate if they fear
