    you do not have your award letter you can get a copy by calling the Social Security Administration at 800 772 1213 IF YOU RECEIVE CASH ASSISTANCE PA DEPARTMENT OF PUBLIC WELFARE Standard letter from the local DPW office stating monthly benefits received A printout showing last 12 months of benefits may also be acceptable IF YOU RECEIVE CHILD SUPPORT Child Support Court Order or a printout reflecting last 12 months of support IF YOU RECEIVE A PENSION IRS Form 1099 or a letter from your pension company showing monthly amounts IF YOU RECEIVE UNEMPLOYMENT Unemployment Determination Notice from the Bureau of Unemployment Compensation or a letter from the Bureau stating monthly benefits received NOTE Bank statements showing direct deposits are not acceptable proof of income 3 A copy of your current bank statements for both checking and savings accounts How Long is the Wait for Services BSRP the average wait for services is approximately three years WAP clients are prioritized based on income age disability energy usage and number of people in the house The average wait for high priority clients is two years AMP the average wait for services is approximately two years Is There any way That Service can be Accelerated Basic Systems Repair Program Heater replacements are accelerated in the winter months Water shut off situations and accumulated sewage situations are accelerated as well as any situation that is deemed imminently dangerous by a PHDC field inspector Weatherization Assistance Program Clients are prioritized based on income age disability energy usage and number of people in the house Higher priority clients are served first Adaptive Modifications Program Because all clients have physical disabilities AMP cases are not accelerated Clients are served on a first come first served basis Is There a Limit to How Much Work Will be Done Basic Systems Repair Program 18 000 is the maximum allowable value of repairs that will be made PHDC inspectors prepare work estimates for critically needed repairs using PHDC s Work Item Price List Work will be approved only for critically needed repairs and the amount can range from 200 to 18 000 If the initial estimate is over 18 000 BSRP will not proceed with the work and the application will be cancelled Weatherization Assistance Program 6 700 is the maximum allowable value of improvements to be made PHDC inspectors conduct energy audits using advanced technologies and testing protocols The improvements will be only to address needed energy conservation repairs which are prioritized If the initial estimate is over 6 700 not including heater repairs or replacement which are addressed separately WAP will not proceed with the work and the application will be cancelled Adaptive Modifications Program 25 000 is the maximum allowable value of modifications to be made PHDC s inspector and occupational therapist develop specifications for modifications that meet program requirements of increasing accessibility and independence If the initial estimate is over 25 000 certain modifications may be eliminated What Services are Provided Basic Systems Repair
